        Patient Online Terms of Use Agreement</h1>
    <p>
        This website (c-clinicals© Site) is an internet-based on-line information and communication
        service made available to you by Cascades Clinical Systems, Inc. (CCS). By accessing
        or using this c-clinicals© site, you agree to be bound by the terms and conditions
        of these Terms of Use. If you do not wish to be bound by these terms of use, you
        may not access or use this c-clinicals© site.

    <p>
        General Disclaimer: Content and medical advice contained on this c-clinicals© site,
        is owned and presented by the physician and medical practice staff licensed to use
        this system and referred to as the "Content Provider".</p>
    <p>
        This c-clinicals© patient portal is a secure, internet-based, on-line information
        and communication service. The personal information that you provide will not be
        shared by CCS with anyone other than the Content Provider unless instructed by you
        or in the event they it is legally subpoenaed. The Client Provider and CCS have
        taken steps to be HIPAA compliant and put great emphasis on the security of all
        data stored in the c-clinicals© database.</p>
    <p>
        General information:</p>
    <ul>
        <li>Please, do not share your login information with anyone as they may have access
            to your medical records.</li>
        <li>If you lose or forget your password and need a new one, we will only provide that
            information to you (your identity will be confirmed by asking identity verifying
            questions).</li>
        <li>The medical information contained in the c-clinicals© portal may include personal
            health information of a sensitive nature. By agreeing to these Terms of Use, you
            are expressly authorizing the Content Provider to include access to such information
            through your portal.</li>
        <li>You are required to provide a valid, personal, email address in order to use the
            portal. The only messages that will be sent to you, by email, are appointment remainders
            and notices to log into your portal account to view new or updated information.
            Should you provide us an email address other than your own, (spouse, child, etc.),
            please make certain that it is someone you trust, as there are risks involved.</li>
        <li>CCS will never disclose your email address to any outside individual or company.
            We strongly encourage patients to use the portal for communication with the physician.
            Use of the portal for communication purposes is voluntary.</li>
        <li>Any patient misuse of the portal may result in their access rights being terminated.</li>
        <li>You are solely responsible for your personal use of this c-clinicals© patient portal
            and agree to hold harmless Cascades Clinical Systems, Inc. for any losses or damages
            you sustain in connection with your personal use or any unauthorized person accessing
            the c-clinicals© Site.</li>
    </ul>
    <h2>
        Use of Content; Ownership</h2>
    <p>
        The entire contents of the Client Provider, c-clinicals© site, constitutes a collective
        work and is the property of the Content Provider which may be protected by copyright
        and other intellectual property laws.</p>
    <h2>
        No Warranty.</h2>
    <p>
        All of the services and information accessed through the c-clinicals© site are provided
        "as is" and without warranty, expressed or implied. CCS makes no warranty as to
        the reliability, accuracy, timeliness, usefulness, adequacy, completeness or suitability
        of the content or the output of the c-clinicals© product. CCS does not warrant against
        human and machine errors, omissions, delays, interruptions or losses, including
        loss of data. CCS does not guarantee or warrant that files available for downloading
        from this online c-clinicals© site will be free of infection by viruses, worms,
        trojan horses or other code that manifest contaminating or destructive properties.
        CCS does not warrant or guarantee that the functions or services performed in the
        c-clinicals© site will be uninterrupted or error free.</p>
    <p>
        Upon acceptance of the terms and conditions contained in this Terms of Use, the
        registered user of the c-clinicals© site is authorized to view or download the material,
        directly pertaining to him or her only, solely for personal, noncommercial use.</p>
    <h2>
        User and Member Representations.</h2>
    <p>
        You represent and warrant to CCS and its officers, directors, employees, agents,
        Content Providers and suppliers that:</p>
    <ol>
        <li>you are at least 18 years of age;</li>
        <li>you possess the legal right and ability to enter into this Agreement and to use
            the c-clinicals© Site in accordance with this Agreement; and</li>
        <li>you shall comply with your responsibilities and obligations as stated in this Agreement.</li>
    </ol>
    <h2>
        Waiver and Release.</h2>
    <p>
        You agree that CCS, nor its officers, directors, employees, agents, or Content Providers shall have any liability
        to you under any theory of liability or indemnity in connection with your use of the c-clinicals© site.
        You hereby release and forever waive any and all claims you may have against CCS, or its officers, directors, employees, or Content Providers
        (short of claims based upon our negligence, or the negligence of our officers, directors, employees, agents, or Content Providers)
        for losses or damages you sustain in connection with your use of the c-clinicals© site.
    </p>
    <h2>
        Indemnity.</h2>
    <p>
        You agree to indemnify, defend and hold harmless CCS, its officers, directors, employees,
        agents, and Content Providers from and against all losses, expenses, damages and
        costs (including attorney’s fees), resulting from any violations of this Agreement
        or any activity related to your account (including infringement of third parties’
        worldwide intellectual property rights or negligent or wrongful conduct) by you
        or any other person accessing the c-clinicals© site using your service account.</p>
